The patient is a 40 year-old IT employee. He complains of a sudden decrease in energy and feeling giddy, requiring to urinate frequently and feeling parched more often. Review of Systems General: Anxious and weak Skin:C lean Head and Neck: No complaints of headaches or neck pain or stiffness. Eyes: No complaints Ears, Nose, Throat and Mouth: Complain with regard to feeling parched Lymphatic System: No node enlargement or tenderness detected. Chest and Lungs: No chest pain, shortness of breath, or cough. Heart and Blood vessels: No complaints. Gastrointestinal and Endocrine System: No complaints of pain Musculoskeletal System: No complaints of muscular pain, joint stiffness or restriction of movements. Nervous System: No complaints.
